A skilled and compassionate Home Health Physical Therapist is needed to deliver patient-centered care in a field-based role. This contract position will allow you to make a direct impact on patient lives by supporting their functional independence within their homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Role Overview: You will complete comprehensive patient evaluationsincluding OASIS assessmentsdevelop and implement individualized care plans, and provide skilled physical therapy interventions such as gait training, transfer training, and therapeutic exercise. Daily responsibilities involve collaborating closely with interdisciplinary healthcare team members, educating patients and caregivers on safety and exercise, and maintaining accurate documentation using Home Care Home Base (HCHB). Expect to see approximately 2532 points weekly, with 57 visits per day, and to regularly travel within your assigned territory (mileage reimbursement provided).

Qualifications:

• Active, unrestricted Physical Therapist license in the state of practice
• Minimum one year of recent home health PT field experience
• OASIS assessment experience required
• Proficiency with Home Care Home Base (HCHB) electronic documentation preferred
• Proven clinical assessment and decision-making abilities
• Reliable transportation, valid drivers license, and current auto insurance
• Strong time management, communication, and organizational skills
• Ability to work autonomously within a flexible, field-based schedule

$2,253 - $2,383/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
