Job Description

Exciting opportunity for an experienced Home Health Physical Therapist to deliver compassionate, patient-centered care in a field-based role. This full-time, 13-week contract focuses on providing skilled physical therapy to patients in their homes, supporting functional mobility and independence with the flexibility of field work.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Complete comprehensive home health physical therapy evaluationsincluding OASIS assessments for Start of Care, Resumption, Recertification, and Discharge when required.
  • Develop individualized care plans targeting functional mobility, strength, balance, gait, and safety.
  • Deliver skilled interventions: therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
  • Educate patients and caregivers on exercise programs, safe mobility, assistive device use, and home safety.
  • Accurately document all care activities and assessments using Home Care Home Base (HCHB).
  • Collaborate closely with nurses, occupational therapists, speech-language pathologists, and home health aides during case conferences and interdisciplinary meetings.
  • Adhere to regulatory, Medicare, and agency standards while ensuring high-quality, coordinated care.

Qualifications:

  • Active, valid PT license in the state of practice
  • Minimum one year of recent home health physical therapy experience
  • Proficiency with Home Care Home Base (HCHB) highly preferred
  • Required OASIS assessment experience
  • Reliable transportation, valid drivers license, and current auto insurance
  • Strong organizational, communication, and clinical decision-making abilities
  • Comfortable working independently in a home health setting
