Job Description
Make a meaningful impact as a Home Health Physical Therapist by providing skilled, compassionate care to patients in a home care setting. This role is ideal for therapists who excel at working independently and thrive on delivering exceptional, patient-focused physical therapy services.
Schedule & Assignment
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Average caseload: 2532 points per week (approximately 57 patient visits per day)
- Daily travel within assigned territory, supported by mileage reimbursement
Key Responsibilities
- Complete comprehensive physical therapy evaluations, including OASIS assessments
- Develop, implement, and adjust individualized care plans focused on functional mobility, fall prevention, gait, pain management, and overall safety
- Deliver skilled therapeutic interventions, including exercise, neuromuscular re-education, and transfer/gait training
- Educate patients and caregivers on home exercise programs, safety, and assistive devices
- Collaborate with interdisciplinary team members (RN, OT, SLP, social workers, aides) to coordinate care
- Maintain timely, accurate documentation using Home Care Home Base (HCHB)
- Participate in case conferences, interdisciplinary meetings, and required continuing education
- Adhere to regulatory standards and agency policies
Qualifications
- Current Physical Therapist (PT) license in good standing
- At least 1 year of recent, field-based home health PT experience
- Proficiency with Home Care Home Base (HCHB) preferred
- OASIS assessment experience required
- Reliable transportation, valid drivers license, and active auto insurance
- Strong communication, clinical decision-making, and time management skills
- Ability to function autonomously and manage a field-based schedule
