An opportunity is available for an experienced Home Health Physical Therapist (PT) to deliver skilled therapy services directly in patients' homes in the Wynne, AR area. This full-time, contract position focuses on improving patient mobility, strength, balance, and overall independence through personalized treatment plans and hands-on interventions in a field-based setting.
Key Responsibilities:
- Conduct comprehensive physical therapy evaluations, including OASIS assessments when required.
- Develop and implement individualized therapy plans targeting functional mobility, gait training, pain management, and safety.
- Provide skilled interventions such as therapeutic exercise, neuromuscular re-education, transfer and fall prevention training.
- Monitor patient progress, update treatment plans, and communicate clinical changes to physicians and care teams.
- Educate patients and caregivers on exercises, safe home practices, and assistive device use.
- Maintain accurate and timely documentation using Home Care Home Base (HCHB) software.
- Collaborate with interdisciplinary team members including nurses, occupational therapists, and social workers.
- Comply with Medicare home health guidelines and agency policies.
Qualifications:
- Active PT license in good standing for the state of practice.
- Minimum one year of recent home health physical therapy experience; field-based experience preferred.
- Proficiency with HCHB documentation and OASIS assessment tools.
- Strong clinical decision-making and functional assessment capabilities.
- Reliable transportation, valid driver's license, and insurance.
- Ability to work independently and manage a flexible home visit schedule.
- Excellent communication, organization, and time management skills.
Schedule & Details:
- Full-time, 13-week contract.
- Monday through Friday, 8:00 AM 5:00 PM.
- Caseload averages 25-32 points weekly, approximately 5-7 patient visits per day.
- Mileage reimbursement provided for travel within assigned territory.
