A skilled Physical Therapist with experience in home health is sought for a full-time contract position in Wrightsville, GA. This opportunity involves providing personalized therapy services to patients in their homes, helping improve their mobility and overall well-being over a 13-week period.

Responsibilities:

  • Conduct thorough initial assessments of patients physical conditions and limitations.
  • Develop and implement individualized treatment plans based on clinical evaluations and physician directives.
  • Administer therapeutic exercises, manual therapy, and modalities to enhance strength and functional independence.
  • Educate patients and families on exercise techniques, transfers, and use of assistive devices.
  • Monitor patient progress and adjust treatment plans as needed.
  • Communicate with physicians, nurses, and other healthcare professionals to coordinate care.
  • Maintain accurate electronic medical records and submit required documentation per regulatory standards.
  • Adhere to ethical, professional, and regulatory guidelines in the provision of care.

Qualifications:

  • Bachelor's or Masters degree in Physical Therapy from an accredited institution.
  • Current license or eligibility for licensure as a Physical Therapist in the relevant state.
  • Experience working in a home health setting.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical record software.
  • Strong clinical reasoning and critical thinking skills.
  • Effective communication and interpersonal skills.
  • Compassionate, patient-focused approach with ability to motivate others.
  • Knowledge of Medicare regulations and documentation requirements for home health.
  • Valid drivers license and reliable transportation to visit patients homes.
