An opportunity is available for a dedicated and skilled Home Health Physical Therapist to provide essential rehabilitation services to patients in their homes. The primary focus is to help individuals regain mobility, strength, balance, and independence through patient-centered treatment plans. This role is ideal for an organized and compassionate PT who enjoys working autonomously and collaborating with a multidisciplinary team.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
Required Qualifications:
- Active and unrestricted Physical Therapist (PT) license in Oregon
- Minimum of one year of recent experience in home health physical therapy (field-based preferred)
- Proficiency with Home Care Home Base (HCHB) electronic documentation system (preferred)
- Hands-on experience conducting OASIS assessments
- Excellent interpersonal, time management, and organizational skills
- Ability to make sound clinical judgments and assess patient needs independently
- Reliable vehicle, valid drivers license, and up-to-date automobile insurance
Key Responsibilities:
- Complete thorough physical therapy evaluations, utilizing OASIS tools when indicated
- Develop and implement customized plans of care to target functional mobility and patient safety
- Deliver skilled interventions including therapeutic exercise, neuromuscular re-education, and fall prevention
- Monitor progress, adapt treatment plans, and communicate timely updates with the interdisciplinary team and physicians
- Educate patients and caregivers on home exercises, safe use of assistive devices, and strategies to enhance independence
- Maintain timely documentation in Home Care Home Base (HCHB)
- Participate in team meetings and foster collaborative patient care
