Take the next step in your physical therapy career by providing skilled, compassionate care as a Home Health Physical Therapist. This contract role focuses on empowering patients within their homes, enhancing mobility, safety, and independence through evidence-based, patient-centered therapies.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive physical therapy assessments, including OASIS evaluations for start of care, resumption, recertification, and discharge as applicable.
• Develop and execute tailored therapy plans aimed at improving functional mobility, strength, balance, gait, and fall prevention.
• Deliver skilled interventions such as neuromuscular re-education, therapeutic exercise, transfer, and gait training.
• Regularly evaluate patient progress and modify care plans in collaboration with the interdisciplinary healthcare team.
• Educate patients and caregivers on therapeutic exercises, safety, use of assistive devices, and strategies for maximizing function at home.
• Maintain clear, timely documentation using Home Care Home Base (HCHB).
• Participate in case conferences and ongoing professional development activities.
• Ensure clinical practice aligns with agency protocols and all Medicare home health regulations.
• Travel each day within your assigned territory; mileage reimbursement provided.

Required Qualifications:

• Active Physical Therapist (PT) license in good standing for the state of practice.
• At least 1 year of recent home health physical therapy experience; field experience highly valued.
• Hands-on experience with OASIS and electronic documentation via Home Care Home Base (HCHB).
• Valid drivers license, reliable transportation, and current auto insurance.
• Strong clinical decision-making, exceptional communication, and effective time-management skills.
• Capable of working independently in a flexible, field-based role.

$2,414 - $2,554/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
