Job Description
Make a direct impact on patient lives by delivering skilled, compassionate physical therapy in a home health setting. This role involves comprehensive in-home evaluations, developing tailored care plans, and fostering patient progress toward greater independence and mobility.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct initial and ongoing physical therapy assessments, utilizing OASIS when required.
- Create and execute individualized plans focusing on functional mobility, gait, balance, strength, pain management, safety, and fall prevention.
- Deliver hands-on interventions, including therapeutic exercise, neuromuscular re-education, and transfer training.
- Track patient progress, adapt care plans, and communicate updates to physicians and the interdisciplinary team.
- Educate patients and caregivers about exercises, use of equipment, and techniques for safety and daily function.
- Accurately document visits in Home Care Home Base (HCHB) system in compliance with all regulatory guidelines.
- Engage in case conferences and collaborate with nurses, therapists, social workers, and aides to ensure optimal care coordination.
- Adhere to all agency and Medicare home health standards and participate in ongoing professional development.
Qualifications:
- Active Physical Therapist (PT) license (state of practice) in good standing.
- Minimum 1 year of recent Home Health Physical Therapy experience.
- Familiarity with Home Care Home Base (HCHB) and OASIS documentation required.
- Strong clinical decision-making and functional assessment skills.
- Demonstrated ability to work independently in the field.
- Reliable transportation, valid drivers license, and auto insurance.
- Excellent communication and organizational abilities.
