An experienced Home Health Physical Therapist (PT) is needed to provide skilled, patient-focused care in a home setting. This full-time, 13-week contract position involves delivering physical therapy services to improve patients mobility, strength, balance, and overall independence. Working primarily within Winterville, NC, the role requires a self-motivated professional comfortable managing their own schedule while coordinating care with interdisciplinary teams.

Schedule & Work Details:

• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points weekly (57 patient visits per day)
• Field-based visits to patients homes within assigned territory
• Use of Home Care Home Base (HCHB) electronic documentation system
• OASIS assessment completion for patient episodes of care
• Mileage reimbursement provided for travel

Key Responsibilities:

• Conduct thorough physical therapy evaluations, including OASIS assessments
• Develop and implement individualized treatment plans targeting mobility, pain management, gait, and safety
• Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, and fall prevention
• Monitor patient progress, adjust treatments, and communicate updates to physicians and team members
• Educate patients and caregivers on exercises, assistive devices, and safety strategies
• Maintain accurate documentation and participate in interdisciplinary meetings
• Comply with agency policies and Medicare home health regulations

Qualifications:

• Active Physical Therapist license in good standing for the state
• Minimum one year recent home health physical therapy experience; field experience preferred
• Proficiency with Home Care Home Base (HCHB) preferred
• Strong OASIS assessment experience required
• Reliable transportation, valid drivers license, and insurance
• Effective communication, organizational, and time management skills
• Ability to work independently in a flexible, patient-focused role
