Energize your practice as a dedicated Home Health Physical Therapist providing exceptional care to patients in their own homes. This full-time, 13-week contract offers an engaging opportunity to foster patient independence, mobility, and quality of life in a supportive, field-based role.

Schedule & Setting:

• Monday through Friday, 8:00 AM 5:00 PM
• Daily travel to patient homes within assigned territory (mileage reimbursement provided)
• Caseload: Approximately 2532 points per week (averaging 57 patient visits daily)
• Utilize Home Care Home Base (HCHB) for electronic documentation and OASIS for comprehensive assessments

What You'll Bring:

• Active, good-standing Physical Therapist (PT) license in the state of practice
• Minimum one year of recent home health physical therapy experience
• OASIS assessment expertise (required)
• Proficiency with HCHB preferred
• Strong skills in functional assessment, clinical decision-making, and independent time management
• Reliable personal vehicle, drivers license, and current auto insurance
• Excellent communication, organization, and patient education abilities

Key Responsibilities:

• Perform thorough physical therapy evaluations and OASIS assessments when required
• Design and execute individualized care plans focused on mobility, balance, gait, strength, and pain management
• Deliver interventions including therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention education
• Monitor patient progress, update care plans, and partner with physicians and the interdisciplinary care team
• Educate patients and caregivers on home exercises, safety, and assistive device use
• Ensure timely, accurate documentation in HCHB and regulatory compliance
