Job Description
An experienced and compassionate Home Health Physical Therapist (PT) is needed to deliver skilled therapy services directly in patients homes. This role focuses on improving mobility, strength, balance, and overall functional independence by conducting thorough evaluations and creating personalized treatment plans. The ideal candidate is self-motivated, organized, and adept at working independently while providing excellent patient-centered care.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations, including OASIS assessments as required.
- Develop and implement individualized treatment plans targeting mobility, strength, gait, pain management, and safety.
- Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait training, transfer training, and fall prevention.
- Monitor patient progress, modify treatment plans accordingly, and communicate changes to physicians and care team members.
- Educate patients and caregivers on exercises, safety methods, assistive device use, and home exercise programs.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB).
- Collaborate with registered nurses, occupational therapists, speech-language pathologists, medical social workers, and home health aides for coordinated care.
- Participate in case conferences, interdisciplinary meetings, and continuing education.
- Comply with agency policies, Medicare guidelines, and regulatory standards.
Qualifications:
- Active PT license in good standing for the practicing state.
- Minimum of 1 year recent home health physical therapy experience; field experience preferred.
- Proficient in Home Care Home Base (HCHB) and OASIS assessment experience required.
- Strong clinical judgment and functional assessment skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Excellent communication, organization, and time management abilities.
- Ability to work independently with a flexible, field-based schedule.
Job Details:
- Contract: Full-time, 13-week duration
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points per week (57 patient visits daily)
- Setting: Patient homes within assigned territory; mileage reimbursement provided
