Job Description
An experienced Home Health Physical Therapist (PT) is needed to deliver high-quality, patient-focused care in home settings. This full-time, 13-week contract position offers a rewarding opportunity to improve patients mobility, strength, and independence through skilled interventions and personalized treatment plans.
Schedule & Location:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based, with daily travel to patient homes within an assigned territory (mileage reimbursement provided)
- Located in Winchester, KY
Key Responsibilities:
- Conduct comprehensive PT evaluations and OASIS assessments
- Develop and implement individualized care plans targeting mobility, strength, balance, gait, pain management, and safety
- Provide skilled physical therapy treatments including therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
- Monitor patient progress and adjust plans accordingly while maintaining clear communication with physicians and the healthcare team
- Educate patients and caregivers on exercises, assistive device use, and home safety strategies
- Maintain accurate, timely documentation in Home Care Home Base (HCHB)
- Collaborate with interdisciplinary team members including nurses, occupational therapists, and social workers to ensure coordinated care
- Comply with agency policies and Medicare home health regulations
Qualifications:
- Active Physical Therapist license in the state of practice, in good standing
- Minimum 1 year recent Home Health PT experience, with field experience preferred
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment experience required
- Reliable transportation, valid drivers license, and auto insurance
- Strong clinical decision-making, organizational, and communication skills
- Ability to work independently with a flexible, patient-centered approach
