Job Description
Deliver impactful rehabilitation care as an experienced Home Health Physical Therapist, providing skilled therapy services to individuals in their homes. This full-time, 13-week contract position focuses on helping patients regain mobility, strength, and independence through personalized treatment plans and collaborative, patient-centered care.
Schedule & Job Details:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract assignment
- Approximately 2532 points per week (57 patient visits per day, depending on visit types)
- Field-based role with daily travel to patient homes (mileage reimbursement provided)
- Utilizes Home Care Home Base (HCHB) for documentation and OASIS for assessments
Qualifications:
- Current, active Physical Therapist license (in good standing)
- At least 1 year of recent experience in Home Health Physical Therapy
- Proficiency with OASIS documentation
- Prior experience with Home Care Home Base (HCHB) preferred
- Reliable transportation, valid drivers license, and required auto insurance
- Excellent assessment, clinical judgment, organizational, and communication skills
- Ability to work independently and efficiently manage a flexible, field-based caseload
Primary Duties:
- Complete comprehensive physical therapy assessments, including OASIS
- Develop and implement individualized care plans to address patient mobility, strength, and safety
- Provide targeted interventions such as therapeutic exercises, gait training, neuromuscular re-education, and fall prevention
- Educate patients and caregivers on home exercises, safety, and assistive devices
- Document care and progress accurately in HCHB
- Collaborate with an interdisciplinary team to ensure optimum patient outcomes
- Participate in professional meetings, case conferences, and ongoing education
