Deliver expert physical therapy care directly in patients homes as a Home Health Physical Therapist (PT). This opportunity focuses on improving patient mobility, strength, balance, and independence by providing skilled, personalized therapy services in a field-based setting.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations, including mandatory OASIS assessments.
• Develop and implement individualized treatment plans emphasizing functional mobility, pain management, gait training, and fall prevention.
• Provide skilled interventions such as therapeutic exercise, neuromuscular re-education, and transfer training.
• Monitor patient progress and adjust care plans accordingly, communicating changes to physicians and care teams.
• Educate patients and caregivers on exercises, safety practices, assistive device usage, and home exercise programs.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary healthcare professionals to ensure coordinated patient care.
• Comply with agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications and Desired Experience:

• Active Physical Therapist license in good standing for the practicing state.
• At least one year of recent Home Health Physical Therapy experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) system and OASIS assessment experience.
• Strong clinical decision-making, organizational, and communication skills.
• Reliable transportation with a valid drivers license and auto insurance.
• Ability to manage a flexible schedule independently in a field-based environment.

Schedule and Contract Details:

• Full-time, 13-week contract.
• Monday through Friday, 8:00 AM to 5:00 PM.
• Caseload averages 2532 points weekly, approximately 57 patient visits per day.
• Mileage reimbursement provided for daily travel within the assigned territory.
