Schedule:

• Full-time, 13-week contract
• MondayFriday, 8:00 AM5:00 PM
• Field-based position, daily travel to patient homes (mileage reimbursement provided)
• Caseload of approximately 2532 points per week (typically 57 visits per day)

Key Responsibilities:

• Complete thorough physical therapy evaluations, including OASIS assessments for start of care, resumption, recertification, and discharge when appropriate
• Design and implement individualized treatment plans that focus on mobility, strength, balance, pain reduction, and safety
• Provide skilled therapy interventions (therapeutic exercises, gait and transfer training, fall prevention, neuromuscular re-education)
• Monitor patient progress, update plans, and communicate effectively with physicians and interdisciplinary team members
• Educate patients and caregivers on exercises, the safe use of assistive devices, and home exercise programs
• Maintain accurate documentation using Home Care Home Base (HCHB) electronic system
• Collaborate in case conferences, interdisciplinary meetings, and ongoing education sessions
• Adhere to clinical guidelines and agency policies

Required Qualifications:

• Active Physical Therapist (PT) license in good standing in the state of practice
• At least 1 year of recent home health PT experience (field experience preferred)
• Experience with OASIS and HCHB (or willingness to train)
• Excellent clinical judgment, assessment, and organizational skills
• Dependable transportation, valid drivers license, and auto insurance

$2,414 - $2,554/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
