Job Description
An experienced Home Health Physical Therapist (PT) is needed to deliver skilled, patient-centered care in home settings within Williamston, NC. This full-time, 13-week contract role involves enhancing patients mobility, strength, balance, and functional independence through comprehensive evaluations and individualized treatment plans.
Schedule and Duties:
- Monday Friday, 8:00 AM 5:00 PM
- Manage approximately 2532 points weekly, equating to 57 patient visits per day
- Conduct thorough physical therapy evaluations using OASIS assessment tools
- Develop and implement care plans emphasizing therapeutic exercise, gait training, fall prevention, and pain management
- Monitor patient progress, adjust treatments, and coordinate care with physicians and interdisciplinary teams
- Educate patients and caregivers on exercises, safety measures, and assistive device use
- Complete timely and accurate documentation using the Home Care Home Base (HCHB) electronic system
- Participate actively in case conferences and comply with all regulatory and agency standards
Qualifications:
- Current, active Physical Therapist license in good standing within the practicing state
- Minimum one year of recent home health physical therapy experience; field experience preferred
- Proficient with OASIS documentation; experience with HCHB preferred
- Strong clinical and functional assessment skills
- Reliable transportation, valid drivers license, and automobile insurance
- Ability to work independently with excellent communication and organizational skills
