Job Description

An experienced Home Health Physical Therapist (PT) is needed to deliver skilled, patient-centered care in home settings within Williamston, NC. This full-time, 13-week contract role involves enhancing patients mobility, strength, balance, and functional independence through comprehensive evaluations and individualized treatment plans.

Schedule and Duties:

  • Monday Friday, 8:00 AM 5:00 PM
  • Manage approximately 2532 points weekly, equating to 57 patient visits per day
  • Conduct thorough physical therapy evaluations using OASIS assessment tools
  • Develop and implement care plans emphasizing therapeutic exercise, gait training, fall prevention, and pain management
  • Monitor patient progress, adjust treatments, and coordinate care with physicians and interdisciplinary teams
  • Educate patients and caregivers on exercises, safety measures, and assistive device use
  • Complete timely and accurate documentation using the Home Care Home Base (HCHB) electronic system
  • Participate actively in case conferences and comply with all regulatory and agency standards

Qualifications:

  • Current, active Physical Therapist license in good standing within the practicing state
  • Minimum one year of recent home health physical therapy experience; field experience preferred
  • Proficient with OASIS documentation; experience with HCHB preferred
  • Strong clinical and functional assessment skills
  • Reliable transportation, valid drivers license, and automobile insurance
  • Ability to work independently with excellent communication and organizational skills
