Deliver impactful patient care in the community by joining a skilled team dedicated to supporting individuals in the comfort of their homes. This contract opportunity calls for an experienced Home Health Physical Therapist skilled in independent, field-based practice and committed to enhancing patient mobility, safety, and functional independence.

Schedule & Assignment Details:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (57 patient visits per day, visit-dependent)
• Daily travel within an assigned territory (mileage reimbursement included)
• Utilizes Home Care Home Base (HCHB) for electronic documentation
• OASIS assessments for Start of Care, Recertification, and Discharge, as needed

Core Responsibilities:

• Conduct comprehensive physical therapy evaluations, including OASIS assessments
• Develop and revise individualized care plans aimed at functional mobility, strength, pain management, and fall prevention
• Deliver skilled interventions: therapeutic exercise, transfer training, gait training, and neuromuscular re-education
• Educate patients and caregivers on exercises, home safety, and equipment use
• Maintain meticulous documentation in HCHB per regulatory and compliance standards
• Collaborate across disciplinesnursing, OT, speech, social workto deliver coordinated care
• Participate in interdisciplinary meetings and ongoing development activities

Requirements:

• Active and unrestricted Physical Therapist license in state of practice
• At least one year of recent home health PT experience (field preferred)
• Competence with Home Care Home Base (HCHB) and required OASIS documentation
• Strong communication, clinical assessment, and organizational skills
• Reliable transportation, valid drivers license, and current auto insurance

$2,247 - $2,377/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
