Schedule/Hours:

• MondayFriday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Responsibilities:

• Conduct detailed physical therapy and OASIS assessments for new admissions, resumptions, recertifications, and discharges as needed
• Develop individualized plans of care targeting functional mobility, strength, gait, and safety
• Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention
• Monitor patient progress and modify therapy plans accordingly, updating physicians and interdisciplinary team members
• Educate patients and caregivers about home exercise programs, assistive device usage, and safety strategies
• Complete required documentation using Home Care Home Base (HCHB) efficiently and accurately
• Engage in case conferences and interdisciplinary meetings, and participate in ongoing education

Qualifications:

• Active Physical Therapist license in good standing
• Minimum one year of recent home health physical therapy experience required
• Proficiency in Home Care Home Base (HCHB) and OASIS assessments
• Strong clinical assessment, decision-making, and communication skills
• Comfortable working independently and managing a field-based schedule
• Reliable transportation, drivers license, and auto insurance required

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
