Job Description
A skilled and compassionate Home Health Physical Therapist is needed to deliver high-quality care directly to patients in their homes. This contract position offers the opportunity to make a significant impact by helping individuals regain mobility, strength, and independence through targeted therapy interventions.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload: Approximately 2532 points per week (typically 57 patient visits per day based on visit types)
Key Responsibilities:
- Complete comprehensive physical therapy evaluations, including utilizing OASIS for Start of Care, Resumption, Recertification, and Discharge visits as needed
- Develop and implement tailored plans of care aimed at improving mobility, balance, pain management, and fall prevention
- Provide skilled therapy interventions, including therapeutic exercises, gait training, neuromuscular re-education, and transfer training
- Monitor patient progress, adjust treatment plans, and communicate effectively with physicians and interdisciplinary care team
- Educate patients and caregivers on exercise regimens, safety strategies, use of assistive devices, and home exercise programs
- Ensure timely, accurate documentation using Home Care Home Base (HCHB)
- Collaborate with other healthcare professionals to coordinate care and participate in case conferences and ongoing education
Qualifications:
- Active PT license in the state of practice in good standing
- Minimum of 1 year recent home health PT experience (field experience preferred)
- OASIS assessment experience required
- Proficiency with Home Care Home Base (HCHB) preferred
- Valid drivers license, reliable transportation, and auto insurance
- Strong clinical reasoning, assessment, and time management abilities
