Job Description

An experienced and compassionate Home Health Physical Therapist (PT) is needed to provide skilled physical therapy services in patient homes. This role focuses on improving patient mobility, strength, balance, and functional independence through individualized care plans and collaborative teamwork. Job Type:
Contract Full-time, 13-week duration
Schedule:

  • Monday to Friday
  • 8:00 AM to 5:00 PM

Key Responsibilities:

  • Conduct thorough physical therapy evaluations and complete OASIS assessments as required.
  • Develop and carry out tailored treatment plans focused on functional mobility, strength, balance, pain management, and safety.
  • Provide skilled interventions including therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
  • Monitor patient progress, adjust plans accordingly, and communicate updates to healthcare team members.
  • Educate patients and caregivers on exercises, safety, assistive devices, and home programs.
  • Document accurately and timely using Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary team members to ensure coordinated patient care.
  • Adhere to agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications/Desired Experience:

  • Active Physical Therapist license in good standing for the state.
  • Minimum one year recent home health physical therapy experience, preferably field-based.
  • Proficiency with Home Care Home Base (HCHB) and OASIS required.
  • Strong clinical decision-making and functional assessment abilities.
  • Reliable transportation with valid driver's license and insurance.
  • Excellent communication, time management, and organizational skills.
