A Home Health Physical Therapist (PT) position is available for a skilled professional to provide patient-centered care in a home setting. This full-time, 13-week contract role involves delivering physical therapy services aimed at improving mobility, strength, balance, and overall independence for patients in their residences. The position requires a self-motivated and organized individual comfortable working independently while collaborating with a multidisciplinary healthcare team.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations and OASIS assessments.
• Develop and execute individualized treatment plans targeting functional mobility, pain management, gait, and safety.
• Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention.
• Monitor patient progress, modify treatment plans as necessary, and communicate changes with physicians and care team members.
• Educate patients and caregivers on exercise regimes, safety protocols, assistive devices, and home programs.
• Maintain accurate documentation using Home Care Home Base (HCHB).
• Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides.
• Participate in interdisciplinary meetings and continuing education.
• Comply with agency policies, Medicare guidelines, and regulatory standards.

Qualifications:

• Active Physical Therapist license in good standing for the state of practice.
• At least 1 year of recent home health PT experience; field experience preferred.
• Experience with OASIS required; proficiency in HCHB preferred.
• Strong clinical decision-making and functional assessment abilities.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong communication, time management, and organizational skills.
• Ability to work independently and manage a flexible, field-based schedule.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Caseload averaging 2532 points weekly (57 patient visits daily)
• Travel required within assigned territory; mileage reimbursement provided
