Make a lasting difference in patients lives as a dedicated Home Health Physical Therapist serving the local community. This rewarding contract opportunity is ideal for an experienced PT seeking a field-based role focused on skilled, holistic care. You will deliver high-quality physical therapy services directly in patients homes, empowering individuals to achieve their highest level of independence and mobility.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time; 13-week assignment
Key Responsibilities:
- Complete thorough patient evaluations, utilizing OASIS assessments when indicated
- Plan and implement individualized therapy interventions for mobility, strength, balance, and safety
- Deliver skilled services such as therapeutic exercises, gait and transfer training, fall prevention, and pain management
- Monitor patient progress, altering care plans as needed and maintaining open communication with the care team and physicians
- Educate patients and caregivers on home programs, safe handling, and use of assistive devices
- Ensure timely, accurate documentation within the Home Care Home Base (HCHB) platform
- Collaborate effectively with nurses, occupational therapists, speech-language pathologists, and other members of the care team
Qualifications:
- Active, unrestricted Physical Therapist license in the state of practice
- At least one year of recent home health PT experience required
- Proficiency in OASIS assessments and Home Care Home Base (HCHB) preferred
- Ability to work independently, demonstrating strong time management and organizational skills
- Reliable vehicle, valid drivers license, and auto insurance
