Job Description
Deliver essential physical therapy care to patients in a home health setting, making a meaningful difference in their recovery and independence. This contract opportunity calls for an experienced Home Health Physical Therapist to manage a diverse caseload, delivering patient-driven care and collaborating across an interdisciplinary team.
Schedule and Setting:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Approximately 57 patient visits per day (2532 points/week)
- Field-based, providing services in patient homes
- Daily regional travel with mileage reimbursement
Qualifications:
- Current, active Physical Therapist (PT) license in good standing
- At least 1 year of recent Home Health PT experience (field experience strongly preferred)
- Mandatory experience with OASIS home health assessments
- Familiarity with Home Care Home Base (HCHB) electronic documentation is an advantage
- Strong skills in functional assessment and clinical decision-making
- Reliable personal vehicle, valid drivers license, and proof of insurance
- Excellent organization, time management, and communication abilities
- Able to work independently and flexibly in a field-based environment
Core Responsibilities:
- Complete thorough physical therapy evaluations, including required OASIS assessments
- Design and implement individualized care plans to address mobility, function, strength, balance, safety, and pain management
- Deliver evidence-based therapeutic interventions: gait training, exercise prescription, neuromuscular re-education, transfer and fall-prevention instruction
- Educate patients and caregivers on exercise routines, safe equipment use, and home safety protocols
- Maintain timely, accurate electronic documentation in HCHB
- Collaborate on care strategies with nurses, therapists, case managers, and aides
- Attend interdisciplinary meetings and participate in continuing education
- Comply with agency, regulatory, and Medicare guidelines
