Job Description

Experienced and compassionate professionals are sought to deliver skilled physical therapy services to patients in a home health setting. This contract role offers the opportunity to help patients improve their mobility, strength, balance, and overall independenceall from the comfort of their homes. The ideal candidate thrives when working independently, is highly organized, and brings a patient-centered approach to care.

Schedule/Hours:

  • Full-time, 13-week contract
  • Monday through Friday, 8:00 AM 5:00 PM
  • Approximately 2532 points per week (typically 57 patient visits per day)
  • Daily local travel within an assigned territory; mileage reimbursement available

Key Responsibilities:

  • Conduct comprehensive physical therapy evaluations, including completion of OASIS assessments when required
  • Develop and implement individualized treatment plans focused on improving functional mobility, strength, balance, and safety
  • Deliver interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
  • Monitor patient progress, adjust plans as necessary, and communicate status updates to the care team
  • Educate patients and caregivers on home exercise programs, safety, and use of assistive devices
  • Maintain detailed and timely documentation using Home Care Home Base (HCHB)
  • Collaborate with an interdisciplinary team to ensure coordinated, quality care
  • Stay current with Medicare and regulatory guidelines, and participate in meetings and ongoing education

Qualifications:

  • Active licensure as a Physical Therapist (PT) in good standing
  • At least 1 year of recent home health physical therapy experience required
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments strongly preferred
  • Reliable transportation, valid drivers license, and auto insurance required
  • Excellent communication, decision-making, and organizational skills
