Deliver personalized, high-quality care as a Home Health Physical Therapist, providing skilled therapy services to patients in their homes. Your focus will be on maximizing mobility, improving balance and strength, and empowering individuals to achieve greater independenceall while collaborating with a compassionate interdisciplinary healthcare team. This 13-week contract position offers a field-based role supporting patients well-being within a comfortable, familiar environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete thorough physical therapy evaluations, including OASIS assessments.
• Design and implement individualized treatment plans focused on functional improvement and safety.
• Deliver skilled interventions such as therapeutic exercise, gait and transfer training, and fall prevention strategies.
• Regularly evaluate progress, update plans, and coordinate communication with physicians and other care team members.
• Educate patients and caregivers on exercises, use of assistive devices, and safety within the home setting.
• Maintain timely, accurate documentation using Home Care Home Base (HCHB).
• Participate in interdisciplinary meetings and continual professional development.
• Uphold agency and regulatory standards, including Medicare requirements.

Qualifications:

• Active Physical Therapist license in good standing in the state of practice.
• At least one year of recent field-based Home Health Physical Therapy experience.
• OASIS experience required; proficiency with Home Care Home Base (HCHB) strongly preferred.
• Excellent clinical judgment, organizational, and communication skills.
• Independent, reliable, and flexibleable to manage a field-based caseload.
• Valid drivers license, auto insurance, and reliable transportation.
