Job Description

Provide compassionate, high-quality physical therapy care to patients in their homes as a Home Health Physical Therapist on a 13-week contract in the Waterbury, CT area. This role involves assessing and developing personalized treatment plans to enhance patient mobility, strength, and independence within the home health setting.

Key Responsibilities:

  • Perform thorough initial assessments to evaluate patients physical status and functional abilities.
  • Design and implement individualized therapy programs based on assessment findings and physician orders.
  • Utilize therapeutic exercises, manual therapy, and modalities to improve patient outcomes.
  • Educate patients and caregivers on exercise techniques, assistive device use, and safe mobility.
  • Monitor patient progress and adjust treatment plans as needed to meet goals.
  • Collaborate with physicians, nurses, and other healthcare professionals to coordinate care.
  • Maintain accurate documentation using electronic medical records, ensuring compliance with regulations.
  • Adhere to ethical standards and regulatory requirements in delivering patient care.

Desired Qualifications:

  • Bachelors or Masters degree in Physical Therapy from an accredited program.
  • Current state licensure or eligibility for licensure as a Physical Therapist.
  • Experience in a home health physical therapy setting preferred.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical records.
  • Strong clinical judgment, communication, and interpersonal skills.
  • Knowledge of Medicare regulations and documentation standards for home health.
  • Valid drivers license and reliable transportation for travel to patient homes.
