Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active Physical Therapist (PT) license in good standing for the state of practice
• At least 1 year of recent Home Health Physical Therapy experience (field-based experience strongly preferred)
• OASIS assessment experience required
• Familiarity with Home Care Home Base (HCHB) electronic documentation preferred
• Strong clinical assessment, critical thinking, and patient-centered care skills
• Reliable transportation with valid drivers license and auto insurance
• Able to work independently, stay organized, and effectively manage a flexible field schedule

Responsibilities:

• Conduct comprehensive patient evaluations, including OASIS for start of care, follow-up, and discharge
• Develop and implement individualized treatment plans to enhance mobility, safety, and quality of life
• Provide skilled therapeutic interventions such as gait training, neuromuscular re-education, and fall prevention in the home setting
• Educate patients and caregivers on home exercises, safety strategies, and use of assistive devices
• Maintain timely and accurate documentation using HCHB
• Collaborate closely with interdisciplinary care teamsnurses, therapists, social workers, and aides
• Participate in case conferences, team meetings, and ongoing professional development
• Ensure regulatory compliance and high-quality standards in all care provided

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
