Provide expert physical therapy care as a Home Health Physical Therapist (PT) delivering patient-centered services in home settings. This contract opportunity offers a full-time, 13-week assignment with a schedule of Monday through Friday, 8:00 AM to 5:00 PM. You will manage a caseload of approximately 2532 points weekly, typically equating to 57 patient visits a day, utilizing your expertise to enhance patient mobility, strength, balance, and independence.

Responsibilities include conducting thorough physical therapy evaluations and OASIS assessments, developing and implementing tailored treatment plans, and delivering skilled interventions such as therapeutic exercises, gait training, and fall prevention techniques. You will monitor patient progress, update care plans accordingly, and engage in effective communication with physicians and interdisciplinary team members. Patient and caregiver education on safety, exercise regimens, and assistive devices is key. Accurate and timely documentation using Home Care Home Base (HCHB) is essential, alongside adherence to agency policies, Medicare guidelines, and participation in case conferences and continuing education.

Qualifications:

• Current Physical Therapist license in good standing for the state of practice
• Minimum of 1 year recent Home Health Physical Therapy experience; field experience preferred
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment experience required
• Strong clinical decision-making and functional assessment capabilities
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently while managing a flexible, field-based schedule
• Excellent communication, organizational, and time management skills
