An exciting opportunity is available for an experienced and compassionate physical therapist to deliver skilled home health services. In this pivotal role, you will support patients recovering or managing chronic conditions as they work to regain functional mobility and independence in their home environments. Schedule and Contract Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points per week (typically 57 visits per day, depending on visit types)
- Daily travel within an assigned territory (mileage reimbursement provided)
- Electronic documentation with Home Care Home Base (HCHB)
- OASIS used for Start of Care, Resumption, Recertification, and Discharge assessments
Key Responsibilities:
- Conduct comprehensive physical therapy evaluations, including OASIS assessments as required
- Develop and implement individualized care plans emphasizing mobility, strength, balance, and safety
- Deliver therapeutic interventions: exercise, neuromuscular re-education, gait and transfer training, and fall prevention
- Track patient progress and adjust care plans in collaboration with physicians and the interdisciplinary team
- Educate patients and caregivers on home exercise programs, safety strategies, and assistive device use
- Maintain timely, accurate documentation in HCHB
- Attend interdisciplinary meetings and participate in ongoing professional development
- Adhere to all regulatory standards, agency policies, and Medicare guidelines
Qualifications:
- Active PT license (in good standing) in state of practice
- Minimum of one year of recent home health PT experience (field experience strongly preferred)
- OASIS experience required; HCHB proficiency preferred
- Ability to work independently, manage flexible schedules, and maintain strong organizational skills
- Reliable transportation, valid drivers license, and auto insurance
