Job Description

Provide expert physical therapy services directly in patients homes as a Home Health Physical Therapist on a 13-week contract in the Waltham, MA area. This full-time role offers an opportunity to deliver personalized care that enhances patients mobility, strength, and independence while collaborating with an interdisciplinary healthcare team.

Key Responsibilities:

  • Conduct thorough initial assessments to evaluate physical conditions, mobility limitations, and functional abilities.
  • Develop and implement customized treatment plans based on evaluations, patient goals, and physician directives.
  • Deliver therapeutic exercises, manual therapy, and modalities tailored to patient needs.
  • Educate patients and their families on exercise techniques, transfers, and proper use of assistive devices.
  • Monitor patient progress and adjust treatment plans accordingly.
  • Maintain clear, timely documentation and electronic medical records compliant with regulatory standards.
  • Communicate effectively with physicians, nurses, and other healthcare professionals to coordinate and optimize patient care.
  • Observe all ethical guidelines and state/federal compliance requirements.

Qualifications:

  • Bachelors or Masters degree in Physical Therapy from an accredited institution.
  • Current state licensure or eligibility to practice as a Physical Therapist.
  • Experience in a home health physical therapy setting.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical record software.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Compassionate, patient-focused approach with motivation skills.
  • Thorough knowledge of Medicare regulations and home health documentation.
  • Valid drivers license and reliable transportation for travel to patient homes.
