Schedule & Assignment Overview:

• Contract length: 13 weeks
• Hours: Monday Friday, 8:00 AM 5:00 PM
• Caseload: Approximately 2532 points/week (57 patient visits per day)
• Setting: Patient homes within assigned local territory
• Documentation: Home Care Home Base (HCHB)
• Assessment: Utilize OASIS for Start of Care, Recertification, and Discharge as appropriate
• Daily travel with mileage reimbursement

Primary Responsibilities:

• Perform comprehensive physical therapy evaluations, integrating OASIS assessments as required
• Develop and execute individualized treatment plans to address functional mobility, strength, safety, and independence at home
• Deliver skilled interventionstherapeutic exercise, gait, neuromuscular re-education, fall prevention, and transfer training
• Track patient progress, adapt care plans as needed, and ensure clear communication with providers and other care team members
• Educate patients and caregivers on exercises, safety, equipment use, and home routines
• Maintain thorough and prompt electronic documentation in HCHB
• Participate in interdisciplinary collaboration and meeting requirements per agency and Medicare guidelines

Qualifications:

• Current Physical Therapist (PT) license in good standing for the state
• At least 1 year of recent field-based Home Health PT experience
• Proficiency with Home Care Home Base (HCHB) strongly preferred
• Required OASIS assessment experience
• Excellent clinical judgment, organization, and communication skills
• Reliable transportation, valid drivers license, and up-to-date auto insurance
• Self-sufficient and able to manage a flexible, independent schedule

$2,253 - $2,383/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
