Seeking a dedicated and knowledgeable physical therapist with expertise in home health to provide essential care to patients in their own residences. This contract position offers a field-based schedule, promoting patient independence and improved quality of life through skilled therapy services.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: Approximately 2532 points per week (typically 57 patient visits daily, depending on visit types)

Key Responsibilities:

• Complete comprehensive physical therapy evaluations, including OASIS assessments for Start of Care, Recertifications, and Discharges when applicable.
• Develop and implement customized therapy plans targeting functional mobility, strength, gait, pain management, and safety.
• Deliver skilled interventions, including therapeutic exercise, neuromuscular re-education, and transfer/fall prevention training.
• Collaborate with interdisciplinary team members (nurses, occupational therapists, SLPs, social workers, aides) for holistic patient support.
• Provide education to patients and caregivers regarding exercises, safety, and equipment usage.
• Maintain timely, accurate documentation using Home Care Home Base (HCHB).
• Attend case conferences and interdisciplinary meetings as required.
• Ensure adherence to agency policies, Medicare, and regulatory standards.

Qualifications:

• Active, unrestricted Physical Therapist (PT) license in state of practice.
• At least 1 year of recent home health physical therapy experience preferred.
• Proficiency with HCHB and required experience with OASIS documentation.
• Excellent clinical decision-making, organization, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Comfortable managing an independent, flexible, field-based schedule.

$1,718 - $1,946/Week Estimated weekly earnings represent hourly wages, based on 32 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
