Provide expert physical therapy care as a Home Health Physical Therapist (PT) serving patients in their residences. This role demands strong clinical skills, independence, and a patient-centered approach to enhance mobility, strength, and overall function for individuals in their home environment.

Key Responsibilities

• Perform comprehensive physical therapy evaluations and OASIS assessments.
• Develop and execute personalized treatment plans targeting mobility, balance, strength, pain management, and safety.
• Deliver skilled interventions including therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention.
• Monitor patient progress and adjust care plans accordingly while collaborating with physicians and care teams.
• Educate patients and caregivers about exercises, safety measures, and assistive device use.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Work closely with interdisciplinary teams to ensure cohesive patient care.
• Adhere to agency policies, Medicare guidelines, and regulatory standards.

Qualifications

• Active Physical Therapist license in good standing in the state of practice.
• Minimum one year of recent home health physical therapy experience, preferably in a field-based capacity.
• Experience with OASIS assessments required; proficiency with HCHB preferred.
• Strong clinical reasoning and functional assessment skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Excellent communication, time management, and organizational capabilities.
• Ability to work independently with a flexible schedule for home visits.

Job Details

• Contract Type: Full-time, 13-week contract
• Schedule: Monday through Friday, 8:00 AM to 5:00 PM
• Caseload: 2532 points weekly, approximately 57 patient visits per day
• Mileage reimbursement provided
