Job Description
Make a difference every day by helping patients regain independence in the comfort of their own homes. This full-time, 13-week contract opportunity is ideal for a skilled and compassionate Physical Therapist passionate about home health. You will provide essential therapy services, delivering individualized care to support patients functional mobility, safety, and overall well-being.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive physical therapy evaluations using OASIS assessments as needed
- Develop, implement, and adjust individualized plans of care for patients with a focus on functional mobility, strength, balance, gait, pain management, and safety
- Deliver evidence-based interventions, including therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention
- Monitor clinical progress and collaborate closely with physicians and interdisciplinary team members
- Educate patients and caregivers on safe exercises, use of assistive devices, and home exercise routines
- Maintain detailed, timely electronic documentation using Home Care Home Base (HCHB)
- Participate in interdisciplinary meetings, case conferences, and ongoing education for continuous improvement
- Ensure adherence to regulatory standards and Medicare home health guidelines
Desired Qualifications:
- Active Physical Therapist (PT) license in good standing
- Minimum one year of recent home health physical therapy experience (field-based preferred)
- OASIS assessment experience required
- Familiarity with Home Care Home Base (HCHB) preferred
- Reliable transportation, valid drivers license, and current auto insurance
- Strong clinical reasoning, autonomous work ethic, and organizational skills
