Job Description

Dedicated to making a difference in patients lives, this role calls for a skilled Home Health Physical Therapist to deliver hands-on rehabilitation in a patients own environment. Working as part of a collaborative care team, youll have the autonomy to assess, plan, and implement effective therapy strategiesempowering individuals to regain independence and improve their quality of life.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Approximately 2532 points per week (57 patient visits per day)
  • Field-based with daily travel to patients homes

Desired Qualifications:

  • Active Physical Therapist license (state of practice) in good standing
  • At least 1 year of recent Home Health Physical Therapy experience
  • Experience conducting OASIS assessments and using electronic documentation platforms (HCHB preferred)
  • Reliable transportation, valid drivers license, and current auto insurance
  • Excellent communication, time management, and organizational skills
  • Strong clinical judgment and independent work ethic

Core Responsibilities:

  • Complete thorough patient evaluations and required OASIS assessments
  • Create, implement, and regularly update individualized care plans focused on mobility, pain management, safety, and functional improvement
  • Deliver skilled PT interventions including therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
  • Educate patients and caregivers on home exercise programs and safe use of assistive devices
  • Document all clinical findings and care thoroughly and efficiently using HCHB
  • Collaborate with nurses, therapists, social workers, and aides to achieve optimal patient outcomes
  • Attend case conferences and interdisciplinary meetings as needed

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
