Provide high-quality, patient-centered physical therapy to individuals in their homes, supporting improved mobility, strength, balance, and independence. This contract position calls for a self-driven, organized professional comfortable working independently across a field-based setting, with a focus on delivering top-tier care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive physical therapy assessments, including OASIS evaluations as required.
• Develop and implement individualized care plans focused on mobility, pain management, balance, and safety.
• Provide skilled therapeutic interventions such as exercise regimens, gait and transfer training, neuromuscular re-education, and fall prevention strategies.
• Assess patient progress, adapt treatment plans as needed, and communicate updates with physicians and interdisciplinary team members.
• Instruct patients and caregivers in home exercise programs, assistive device use, and safety techniques.
• Maintain timely, accurate documentation using the Home Care Home Base (HCHB) system.
• Engage in collaborative care planning with registered nurses, therapists, social workers, and aides.
• Participate in meetings, continuing education sessions, and ensure compliance with agency policies and Medicare home health guidelines.

Desired Qualifications:

• Current, valid Physical Therapist (PT) license (in good standing).
• At least 1 year of recent home health PT experience; field-based experience preferred.
• Proficiency with Home Care Home Base (HCHB) strongly preferred.
• Required experience with OASIS assessments.
• Demonstrated clinical competency, strong judgment, organizational, and time management skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and maintain a flexible field-based schedule.
• Excellent communication and teamwork abilities.
