Job Description
Deliver skilled physical therapy care directly to patients in a home health setting, focusing on improving mobility, strength, and overall independence. This full-time, 13-week contract position calls for a detail-oriented clinician ready to provide compassionate, patient-centered support while working independently in the field.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Approximately 2532 points per week (typically 57 patient visits per day)
Key Responsibilities:
- Perform comprehensive physical therapy evaluations, including OASIS assessments as needed.
- Develop and execute individualized plans of care targeting mobility, safety, and functional gains.
- Deliver evidence-based interventions, such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention.
- Track patient progress and collaboratively adjust care plans, communicating updates to physicians and interdisciplinary team members.
- Instruct patients and caregivers in safe practices, assistive devices, and home exercise regimens.
- Complete thorough, timely documentation using Home Care Home Base (HCHB).
- Collaborate with nursing, OT, SLP, social work, and aide personnel.
- Participate in team meetings, ongoing education, and adhere to all compliance and regulatory guidelines.
Qualifications:
- Active, unencumbered Physical Therapist (PT) license in the state
- At least 1 year of recent experience in home health physical therapy
- Proficiency in electronic documentation (Home Care Home Base/HCHB preferred)
- Required OASIS experience
- Reliable transportation, valid drivers license, and proof of auto insurance
- Strong communication, organizational, and time management abilities
- Ability to work autonomously within a flexible, field-based schedule
