As a Home Health PT, you will utilize your clinical expertise to support recovery, improve independence, and enhance quality of life for diverse patients in their home environment. This contract role offers the autonomy to manage your schedule within an assigned territory and collaborate with multidisciplinary colleagues dedicated to high-quality, patient-centered care.

Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: Approximately 2532 points weekly (57 patient visits/day)
• Field-based with daily travel (mileage reimbursement provided)

Key Responsibilities:

• Complete comprehensive physical therapy evaluations, including OASIS assessments when needed
• Develop and implement individualized plans of care targeting functional mobility, safety, and independence
• Deliver skilled therapeutic interventions including exercise, patient education, gait and transfer training, and fall prevention
• Monitor progress, update care plans, and communicate with physicians and team members
• Educate patients and caregivers on home safety, exercise, and device use to support ongoing recovery
• Maintain timely, accurate documentation using Home Care Home Base (HCHB)
• Collaborate with RNs, OTs, SLPs, social workers, and home health aides for coordinated care delivery
• Attend case conferences and ongoing training as required

Qualifications:

• Active Physical Therapist (PT) license in the state of practice
• At least 1 year of recent home health physical therapy experience
• Experience with OASIS and proficiency in HCHB preferred
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and manage a field-based caseload

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
