Job Description

Key Details:

  • 13-week contract, full-time assignment
  • Monday through Friday schedule, 8:00 AM 5:00 PM
  • Caseload of approximately 2532 points per week (typically 57 patient visits per day)
  • Field-based role with daily travel to patients homes (mileage reimbursement provided)
  • Utilization of Home Care Home Base (HCHB) for electronic documentation
  • OASIS assessments for Start of Care, Resumption, Recertification, and Discharge, as applicable

Responsibilities Include:

  • Performing comprehensive physical therapy evaluations, including required OASIS assessments
  • Creating and administering individualized treatment plans focusing on mobility, strength, balance, gait, and overall functional independence
  • Delivering skilled interventions such as exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies
  • Monitoring patient progress, updating care plans, and communicating updates to physicians and interdisciplinary team members
  • Educating patients and caregivers about safety, exercise programs, assistive devices, and preventive strategies
  • Ensuring timely, accurate documentation within the HCHB system
  • Collaborating with nurses, occupational therapists, speech-language pathologists, medical social workers, and aides
  • Participating in case conferences, interdisciplinary meetings, and required ongoing education
  • Adhering to regulatory requirements and Medicare home health standards

Qualifications:

  • Active state Physical Therapist (PT) license (in good standing)
  • At least 1 year of recent home health PT experience preferred
  • Proficiency with Home Care Home Base (HCHB) preferred
  • Required OASIS assessment experience
  • Effective clinical, communication, and organizational skills
  • Reliable transportation, valid drivers license, and auto insurance
