Job Description

Providing expert in-home physical therapy services, this role involves working directly with patients to enhance mobility, strength, balance, and overall independence. As a Home Health Physical Therapist, you will evaluate patients comprehensively, craft tailored treatment plans, and closely monitor progress while ensuring adherence to physician directives and care standards.

Key Responsibilities:

  • Perform thorough physical therapy evaluations including OASIS assessments as required.
  • Create and implement personalized treatment plans addressing mobility, strength, pain management, gait, balance, and safety.
  • Deliver skilled interventions such as therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention.
  • Assess patient progress, adjust treatments accordingly, and communicate updates to physicians and multidisciplinary teams.
  • Educate patients and caregivers on exercises, safety protocols, assistive devices, and home programs.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Collaborate with healthcare team members including nurses, occupational therapists, speech therapists, social workers, and aides.
  • Participate in case conferences, interdisciplinary meetings, and continuing education activities.
  • Comply with agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications and Experience:

  • Active Physical Therapist license in good standing in the state of practice.
  • Minimum of 1 year recent experience in Home Health Physical Therapy; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) preferred.
  • Experience with OASIS assessments required.
  • Strong clinical judgment and functional assessment skills.
  • Reliable transportation, valid drivers license, and insurance required.
  • Ability to work independently with strong organizational and time management skills.
  • Excellent communication skills.

Location: Located in Upper Marlboro, MD, this position offers the opportunity to serve patients in their homes within a dynamic and growing community. Upper Marlboro combines suburban tranquility with convenient access to metropolitan amenities, providing a desirable environment for professionals dedicated to home health care.

Schedule and Details:

  • Full-time, 13-week contract
  • Monday through Friday, 8:00 AM 5:00 PM
  • Caseload averaging 2532 points per week (about 57 patient visits daily)
  • Daily travel within assigned territories (mileage reimbursement provided)
  • Electronic documentation using HCHB
