Deliver high-quality care as a Home Health Physical Therapist, supporting patients in their journey to improved mobility and independence within the comfort of their own homes. As a dedicated clinician, you will leverage your expertise to conduct thorough assessments, create individualized care plans, and deliver skilled therapy interventions for diverse patient needs.

Schedule & Assignment Highlights:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Approximately 2532 points per week (typically 57 patient visits per day)
• Field-based care, with daily travel in the assigned territory

Key Qualifications:

• Active Physical Therapist (PT) license in Pennsylvania, in good standing
• At least one year of recent field-based Home Health PT experience
• OASIS assessment experience required
• Proficiency in Home Care Home Base (HCHB) preferred
• Strong clinical reasoning and patient assessment skills
• Excellent time management, organizational, and communication abilities
• Ability to work independently while ensuring quality, patient-centered care
• Reliable transportation, valid drivers license, and current auto insurance

What Youll Do:

• Conduct comprehensive physical therapy assessments, including OASIS when applicable
• Develop and execute tailored treatment plans focusing on mobility, strength, balance, and functional independence
• Implement effective interventions such as gait and transfer training, therapeutic exercises, and fall prevention strategies
• Monitor and document patient progress using HCHB
• Collaborate with physicians and the interdisciplinary care team for coordinated service delivery
• Educate patients and caregivers on rehabilitation exercises, assistive devices, and home safety practices
• Participate in case conferences, interdisciplinary meetings, and ongoing professional development

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
