As an experienced Home Health Physical Therapist, you will utilize your clinical expertise and independent judgment to deliver goal-oriented care, promote mobility, and empower patients toward greater independence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Desired Qualifications:

• Active Physical Therapist (PT) license in good standing
• Minimum of one year recent field-based Home Health PT experience
• Solid background with OASIS (required) and Home Care Home Base (preferred)
• Strong clinical skills in evaluation, functional assessments, and interventions
• Excellent communication, time management, and organization skills
• Valid drivers license, reliable vehicle, and appropriate insurance
• Self-directed, adaptable, and comfortable managing a flexible caseload

Primary Responsibilities:

• Perform thorough physical therapy assessments (including OASIS documentation)
• Design, implement, and adapt individualized therapy plans improving mobility, strength, balance, and safety
• Deliver skilled interventions: therapeutic exercises, gait and transfer training, neuromuscular re-education, and fall prevention
• Monitor patient progress, update care plans, and communicate with physicians and team members as needed
• Educate patients and caregivers on exercises, home safety techniques, and use of assistive devices
• Ensure quality, accurate, and timely electronic documentation using Home Care Home Base
• Engage in interdisciplinary collaboration, team conferences, and ongoing professional development
• Comply with regulatory, Medicare, and agency procedures

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
