Job Description
Deliver essential, high-quality care as a Home Health Physical Therapist, providing vital support to individuals directly in their homes. Collaborate with a dedicated interdisciplinary team to help patients achieve optimal mobility, independence, and safety through evidence-based physical therapy interventions.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Duties:
- Perform comprehensive physical therapy evaluations, including mandated OASIS assessments.
- Develop personalized care plans addressing mobility, strength, balance, gait, pain management, and fall prevention.
- Deliver skilled therapeutic exercises, neuromuscular re-education, gait and transfer training, and patient safety instruction.
- Closely monitor patient progress, adapting treatment goals and interventions as necessary.
- Provide hands-on education to patients and caregivers about home exercise programs, safe mobility strategies, and assistive devices.
- Accurately document patient outcomes and daily visits using Home Care Home Base (HCHB).
- Communicate changes and coordinate care with physicians, nurses, occupational and speech therapists, social workers, and aides.
- Attend team meetings, case conferences, and required continuing education.
- Follow agency protocols and comply with all Medicare home health regulations.
Required Qualifications:
- Valid Physical Therapist (PT) license in the state of practice, in good standing.
- At least 1 year of recent home health experience as a physical therapist.
- Hands-on proficiency with OASIS; experience with HCHB highly desired.
- Strong assessment, clinical judgment, and organizational skills.
- Capable of working independently with effective time management.
- Reliable transportation, valid drivers license, and up-to-date auto insurance.
