Make a direct impact in patients lives as a dedicated Home Health Physical Therapist. In this rewarding role, youll deliver skilled physical therapy services within patients homes, fostering functional independence and maximizing mobility in a supportive, one-on-one environment.

Schedule & Position Details:

• Full-time, 13-week contract
• Monday through Friday, 8:00 AM 5:00 PM
• Field-based role, providing care in patient homes
• Caseload: Approximately 2532 points weekly (typically 57 visits per day)
• Daily travel within assigned territory with mileage reimbursement

Responsibilities:

• Complete comprehensive physical therapy evaluations, including OASIS assessments as needed
• Develop and execute individualized care plans targeting strength, balance, gait, mobility, and safety
• Implement interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies
• Monitor patient progress, adapt plans as needed, and communicate updates to physicians and team members
• Educate patients and caregivers regarding exercise, safety precautions, home equipment, and ongoing plans for self-care
• Maintain timely, accurate documentation in the Home Care Home Base (HCHB) electronic system
• Collaborate with an interdisciplinary team to ensure seamless patient care and participate in care meetings
• Follow agency policies and comply with Medicare and regulatory guidelines

Desired Experience & Qualifications:

• Current Physical Therapist (PT) license in good standing
• Minimum of one year of recent home health physical therapy experience
• OASIS documentation experience required; proficiency with HCHB preferred
• Strong clinical assessment, communication, and organization skills
• Reliable transportation with valid drivers license and auto insurance
• Ability to work independently and manage a flexible schedule
