Bring your expertise in physical therapy to a rewarding role focused on delivering individualized care to patients in the comfort of their homes. As a Home Health Physical Therapist, youll assess patient needs, devise customized treatment plans, and support each clients journey toward improved mobility, independence, and quality of life.
Schedule:
- Full-time, 13-week contract
- Monday through Friday, 8:00 AM 5:00 PM
- Approximately 2532 points per week (typically 57 patient visits per day based on visit type)
- Field-based with daily travel within your assigned territory
Key Responsibilities:
- Perform thorough physical therapy evaluations, including required OASIS assessments.
- Develop and adjust care plans based on functional goals, patient progress, and physician orders.
- Deliver interventions such as therapeutic exercise, neuromuscular re-education, transfer and gait training, and fall prevention strategies.
- Educate patients and caregivers on safety, use of assistive devices, and home exercise programs.
- Document care accurately and efficiently using Home Care Home Base (HCHB).
- Collaborate closely with nurses, occupational and speech therapists, social workers, and aides to ensure coordinated, high-quality care.
- Participate in interdisciplinary meetings, case conferences, and ongoing professional training.
Qualifications:
- Current, active Physical Therapist (PT) license in good standing for the state of practice
- At least 1 year of recent field-based home health PT experience
- Required experience with OASIS; proficiency in Home Care Home Base (HCHB) preferred
- Effective independent work habits and organizational skills
- Reliable transportation, valid drivers license, and auto insurance
