A skilled and compassionate Physical Therapist is needed to deliver high-quality home health therapy services for a 13-week contract assignment. This field-based opportunity involves providing patient-centered interventions that promote mobility, independence, and optimal well-being for individuals in their homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Qualifications:

• Active Physical Therapist (PT) license in good standing
• At least 1 year of recent experience in Home Health Physical Therapy (field experience highly preferred)
• Familiarity with Home Care Home Base (HCHB) is a plus
• Required proficiency with OASIS assessments
• Reliable transportation, valid drivers license, and proof of auto insurance
• Strong clinical assessment, decision-making, and communication skills
• Ability to work autonomously while managing a flexible, mobile caseload

Key Responsibilities:

• Complete comprehensive, OASIS-based physical therapy evaluations and establish individualized plans of care
• Deliver skilled interventions, including therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention strategies
• Monitor patient progress, update care plans, and communicate changes to physicians and the care team as needed
• Educate patients and caregivers on exercises, home safety, and the use of assistive devices
• Ensure timely, accurate documentation in Home Care Home Base (HCHB)
• Collaborate with nurses, occupational and speech therapists, medical social workers, and home health aides to achieve coordinated patient care
• Participate in case conferences, meetings, and ongoing professional development
• Adhere to regulatory and Medicare guidelines for home health
