Job Description

Step into a rewarding opportunity delivering transformative care as a Home Health Physical Therapist. This contract role allows you to make a direct impact by supporting patients in their own homes, focusing on restoring and optimizing their mobility, independence, and overall quality of life.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM

Youll work with an average caseload of 2532 points per week, typically providing 57 patient visits daily based on visit types. As a valued member of the interdisciplinary care team, youll utilize assessment tools such as OASIS and document thoroughly using Home Care Home Base (HCHB). Commitment to safe, patient-centered practice and consistent communication is key.

Key Responsibilities:

  • Conduct comprehensive physical therapy evaluations, including required OASIS assessments.
  • Design and implement individualized treatment plans to promote functional mobility, safety, strength, and balance.
  • Deliver evidence-based interventions, such as therapeutic exercises, neuromuscular re-education, transfer and gait training, and fall prevention strategies.
  • Monitor, document, and adjust plans based on patient progress; communicate status updates to physicians and care teams.
  • Provide education and support to patients and caregivers about exercises, safety protocols, and home programs.
  • Participate actively in case conferences and interdisciplinary meetings.

Qualifications:

  • Current Physical Therapist (PT) license in good standing
  • At least one year of recent home health physical therapy experience
  • Proficiency in HCHB preferred; OASIS experience required
  • Reliable transportation, active drivers license, and auto insurance
  • Exceptional communication, clinical reasoning, and organizational skills
