Job Description
Provide compassionate, expert care to patients where they are most comfortabletheir own homes. This contract role as a Home Health Physical Therapist involves delivering high-quality, individualized therapy focusing on mobility, strength, and functional independence in a field-based setting. Youll work autonomously, closely collaborating with an interdisciplinary healthcare team to help patients reach their goals.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct detailed physical therapy evaluations, including OASIS assessments as required.
- Develop and implement personalized treatment plans targeting improved mobility, strength, balance, and safety.
- Deliver interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies.
- Reassess progress, modify plans as needed, and maintain clear communication with physicians and care team members.
- Educate patients and caregivers regarding safe practices, exercises, and the use of assistive devices.
- Complete timely and accurate documentation using Home Care Home Base (HCHB).
- Participate in interdisciplinary case conferences and ongoing professional development.
- Adhere to agency and Medicare guidelines.
Qualifications:
- Active Physical Therapist (PT) license in good standing for the state of practice.
- At least 1 year of recent field-based Home Health PT experience.
- Proficiency with Home Care Home Base (HCHB) preferred.
- OASIS assessment familiarity required.
- Strong assessment, clinical judgment, and communication skills.
- Reliable transportation, valid drivers license, and current auto insurance.
