Job Description
Deliver high-quality care as a Home Health Physical Therapist, providing one-on-one support to patients in their homes. This crucial role centers on enhancing functional independence for individuals recovering from illness, injury, or managing chronic conditions.
Schedule & Details:
- Contract position, 13 weeks in duration
- Full-time hours; MondayFriday, 8:00 AM to 5:00 PM
- Field-based visits to patient homes, with a caseload averaging 2532 points per week (typically 57 patient visits per day)
- Mileage reimbursement for work-related travel
Required Skills & Experience:
- Active and unrestricted Physical Therapist (PT) license in Pennsylvania
- At least 1 year of recent home health physical therapy experience
- OASIS assessment tool expertise (for Start of Care, Resumption, Recertification, and Discharge)
- Proficiency with Home Care Home Base (HCHB) electronic documentation preferred
- Excellent judgment, communication, and time management skills
- Valid drivers license, reliable personal transportation, and current auto insurance
Key Responsibilities:
- Conduct comprehensive assessments and complete all OASIS documentation as required
- Create and manage individualized care plans that focus on mobility, strength, balance, safety, and functional goals
- Deliver evidence-based interventions including gait training, transfer techniques, fall prevention, and therapeutic exercise
- Track patient progress, updating plans and collaborating closely with physicians and the interdisciplinary care team
- Educate patients and their caregivers on best practices, safety techniques, and home exercise programs
- Maintain detailed and timely records in the HCHB system
- Participate in case conferences and required meetings to promote consistent care and continuous improvement
