Deliver skilled physical therapy services directly to patients homes, supporting optimal independence, mobility, and quality of life as a dedicated Home Health Physical Therapist. This full-time, 13-week contract opportunity offers a Monday through Friday schedule (8:00 AM 5:00 PM) and an engaging, field-based caseloadtypically 2532 points per week (57 visits daily, based on patient needs). Youll utilize Home Care Home Base (HCHB) for documentation and OASIS assessment tools for care foundation, providing meaningful interventions where they matter most.

Primary Responsibilities:

• Conduct comprehensive physical therapy evaluations, including OASIS assessments for new and existing patients.
• Develop, implement, and regularly modify individualized care plans to enhance functional mobility, strength, balance, gait, and safety.
• Deliver skilled therapy interventionstherapeutic exercise, neuromuscular re-education, gait and transfer training, fall prevention, and pain management.
• Track patient progress, adjust care as clinically indicated, and promptly communicate status updates to the interdisciplinary care team.
• Instruct patients and caregivers on home exercise programs, safe mobility, use of assistive devices, and injury prevention.
• Maintain accurate, timely documentation within HCHB.
• Participate in case conferences and ongoing education, upholding regulatory standards and Medicare guidelines.

Preferred Qualifications:

• Current Physical Therapist (PT) license in good standing for the state.
• At least 1 year of recent, relevant field-based Home Health PT experience.
• Proficiency in using Home Care Home Base (HCHB) and required OASIS expertise.
• Outstanding communication, organization, and time management skills.
• Reliable transportation, valid drivers license, and auto insurance.

$2,727 - $2,885/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
