A rewarding opportunity for an experienced Home Health Physical Therapist (PT) is available in Timmonsville, SC. This full-time, 13-week contract role involves delivering skilled physical therapy services to patients in their homes, with the goal of enhancing mobility, strength, balance, and overall functional independence.

Key Responsibilities:

• Perform detailed physical therapy evaluations and OASIS assessments as required.
• Develop and execute individualized treatment plans addressing functional mobility, gait, pain management, and safety.
• Provide skilled interventions including therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention.
• Monitor patient progress, adjust treatment plans as needed, and communicate updates to physicians and the care team.
• Educate patients and caregivers on exercises, safety measures, assistive device use, and home exercise programs.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
• Participate in case conferences, meetings, and ongoing education.
• Comply with agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications:

• Active Physical Therapist license in the practicing state, in good standing.
• Minimum one year of recent Home Health Physical Therapy experience; field experience preferred.
• OASIS assessment experience is required; proficiency with HCHB preferred.
• Strong clinical decision-making and functional assessment abilities.
• Reliable transportation with valid drivers license and auto insurance.
• Ability to work independently and manage a flexible schedule.
• Excellent communication, organization, and time management skills.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Approximately 2532 weekly points; typically 57 patient visits daily.
