Job Description
Deliver skilled physical therapy services in patients homes as a Home Health Physical Therapist (PT) specializing in improving mobility, strength, balance, and overall functional independence. This contract role offers the opportunity to work independently while providing high-quality, patient-centered care within a structured Monday through Friday schedule.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations and OASIS assessments as required.
- Develop and implement individualized treatment plans addressing gait, pain management, safety, and functional improvements.
- Provide skilled interventions including therapeutic exercises, neuromuscular re-education, gait and transfer training, and fall prevention.
- Monitor patient progress, adjust care plans accordingly, and communicate changes with physicians and the healthcare team.
- Educate patients and caregivers on exercises, safety measures, assistive devices, and home exercise programs.
- Maintain accurate and timely electronic documentation using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary team members to ensure coordinated patient care.
- Adhere to agency policies and Medicare home health guidelines.
Qualifications:
- Active Physical Therapist license in good standing within the state.
- Minimum of one year recent home health physical therapy experience; field experience preferred.
- Proficiency in Home Care Home Base (HCHB) and OASIS assessment experience required.
- Strong clinical decision-making and functional assessment abilities.
- Valid drivers license, reliable transportation, and auto insurance.
- Excellent communication, organization, and time management skills.
- Ability to manage an independent, flexible, field-based schedule.
Schedule & Contract Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: Approximately 2532 points weekly (57 patient visits daily)
- Daily travel within assigned territory with mileage reimbursement provided
