Deliver high-impact home-based care to patients as an experienced Home Health Physical Therapist. This contract position offers the opportunity to improve mobility, strength, and independence for individuals in the comfort of their homes. Success in this role requires autonomy, strong organizational skills, and a commitment to quality, patient-centered support.
Schedule and Assignment Details:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Field-based with daily travel to patient homes (mileage reimbursement provided)
- Caseload: Approximately 2532 points per week (57 patient visits daily)
- Electronic documentation utilizing Home Care Home Base (HCHB)
- OASIS for required assessments
Role Responsibilities:
- Complete thorough physical therapy evaluations, including OASIS assessment when indicated
- Design and implement individualized therapy plans to address mobility, strength, balance, pain management, and functional safety
- Deliver skilled interventions (therapeutic exercise, gait, transfer, and fall prevention training)
- Monitor patient progress and adjust treatment as needed, maintaining open communication with the care team and physicians
- Guide patients and caregivers in safe exercises, assistive device use, and home programs
- Ensure accurate and timely completion of documentation in HCHB
- Actively participate in care conferences, interdisciplinary meetings, and continuing education
- Adhere to all regulatory standards and agency policies
Qualifications:
- Current Physical Therapist (PT) license, with good standing in the state of practice
- Minimum of 1 year of recent home health physical therapy (preferring field experience)
- OASIS assessment expertise required
- Comfortable using Home Care Home Base (preferred)
- Reliable transportation, drivers license, and auto insurance
- Strong independent time management and outstanding communication
