Job Description
An experienced Home Health Physical Therapist (PT) is needed to deliver high-quality therapy services to patients in their homes throughout Tampa, FL. This full-time, 13-week contract role offers the opportunity to enhance patient mobility, strength, balance, and independence through skilled interventions and collaborative care.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations, including OASIS assessments (Start of Care, Resumption, Recertification, Discharge).
- Develop and execute individualized plans of care targeting functional mobility, gait training, pain management, transfer training, and fall prevention.
- Provide skilled therapeutic exercises and neuromuscular re-education tailored to patient needs.
- Monitor patient progress, adjust treatment plans accordingly, and communicate changes to physicians and the interdisciplinary care team.
- Educate patients and caregivers on exercises, safety protocols, assistive device use, and home exercise programs.
- Maintain accurate documentation using Home Care Home Base (HCHB) electronic records.
- Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides.
- Participate in case conferences, team meetings, and continuing education.
- Follow agency policies and Medicare home health regulations.
Qualifications:
- Active Physical Therapist license in the state of practice, with good standing.
- Minimum one year of recent home health physical therapy experience, preferably in the field.
- Experience with OASIS required; proficiency with Home Care Home Base (HCHB) preferred.
- Strong clinical assessment and decision-making skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently and manage a flexible field-based schedule.
- Excellent communication, organization, and time management abilities.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Approximately 5-7 patient visits daily (2532 points/week)
