Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Field-based, providing care in patient homes
• Approximately 2532 points per week (57 patient visits per day)
• Daily travel within your assigned territory (mileage reimbursement provided)

Required Qualifications:

• Active Physical Therapist (PT) license (in good standing)
• At least 1 year of recent Home Health Physical Therapy experience
• OASIS assessment experience (Start of Care, Resumption, Recertification, Discharge)
• Proficiency with Home Care Home Base (HCHB) preferred
• Reliable transportation, valid drivers license, and current auto insurance
• Excellent clinical judgment, communication, and organization
• Ability to work independently and manage a flexible schedule

Responsibilities:

• Complete comprehensive physical therapy evaluations, including OASIS assessments when required
• Develop and implement tailored treatment and home exercise programs for diverse patient needs
• Provide skilled interventions including therapeutic exercises, gait training, and transfer training
• Educate patients and caregivers on safety strategies and the effective use of assistive devices
• Collaborate with nurses, therapists, and social workers for coordinated patient care
• Maintain accurate, timely documentation and participate in team conferences and ongoing education
• Uphold regulatory standards and Medicare home health guidelines

$1,609 - $1,702/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
