Job Description
A skilled and compassionate physical therapist is needed to deliver exceptional care to patients receiving home health services. This full-time, 13-week contract position centers on providing evidence-based rehabilitative care to support patient mobility, safety, and functional independence through individualized interventions in the home setting.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete thorough physical therapy evaluations, including OASIS assessments as needed
- Develop and implement individualized care plans focusing on mobility, strength, balance, gait, pain management, and fall prevention
- Deliver skilled PT interventions such as therapeutic exercises, neuromuscular re-education, transfer/gait training, and patient/caregiver education
- Monitor clinical progress, revise treatment plans, and communicate changes to physicians and healthcare team members
- Ensure accurate, timely documentation using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary team members for high-quality, coordinated patient care
- Attend case conferences, interdisciplinary meetings, and mandatory continuing education
- Comply with agency policies, Medicare guidelines, and regulatory standards
- Travel to patient homes daily within the assigned territory; mileage reimbursement provided
Qualifications:
- Valid Physical Therapist (PT) license in the state of practice (in good standing)
- At least 1 year of recent home health physical therapy experience strongly preferred
- OASIS assessment experience required
- Proficiency with Home Care Home Base (HCHB) preferred
- Clinical expertise in functional assessment and strong decision-making skills
- Reliable transportation, current drivers license, and auto insurance
- Capable of working both independently and as part of a collaborative team
- Excellent written and verbal communication abilities
